What You'll Do: 

• Assess client needs and develop individualized care plans to ensure optimal health outcomes
• Coordinate with physicians, therapists, and caregivers to implement and monitor these plans
• Educate clients and their families on disease management, medications, and care strategies
• Ensure compliance with medical standards and maintaining accurate client records

What You'll Need: 

• Active MN Registered Nurse License
• Minimum of one year of home care experience
• Minimum of two years of nursing experience
• Must have a valid driver’s license and auto insurance
